Licensure Information

Medical licensure is provided by each individual state. The Federation of State Medical Boards maintains a list of State Medical Boards. An application for licensure as a physician or surgeon must be made with the individual state medical board. Usually, these applications can be found at the state medical board Web site. The specific requirements and costs for applications vary from state to state and inquiries must be made with the individual state medical board. The initial licensing fee can be quite expensive. Some internship and/or residency programs will pay for the cost of your application and/or initial licensing fee. The initial licensing fee is not tax deductible, but subsequent license renewals are tax deductible.

Once you have completed the state medical board licensing process, you may then register with the Drug Enforcement Administration in order to prescribe controlled substances. The DEA Drug Registration website contains information for applicants along with an application.
